Student or Patrol/Visitor Accident/Incident Report Short Version (First Aid)

A student or patron/visitor accident report is to be completed for any injury or accident that occurs during a school activity or on the school bus. For general first aid purposes only, complete this short version of the accident report.  If ever there is a question whether to complete a full report, it is better to make a full report for future reference in order to defend possible liability charges. The Human Resource Office, Benefit Coordinator or the business office, Administrative Assistant to the Associate Superintendent should be notified immediately by telephone in all cases of serious injury, with the full report filed as usual.
