Radio Repair Work Order

This form is used to generate a work request for portable radios to be repaired.  Please complete each section of the Radio work order form and submit.  You will receive a call from HiTech SMR about the radio repairs and possible troubleshooting tips.  Before completing this work request, be sure to test the battery and/or the antenna of the radio having trouble. 

Include the model of the radio unit (Found by taking off the battery and looking on radio)
Include the Serial Number of the radio unit (Found by taking off the battery and looking on radio)
(Found by taking off the battery and looking on battery for information) Various radios have a few different batteries that work and we must have the exact battery model and serial number.
