Immunizations Requirements

School Year Immunization Requirements 

  • Pre-K – 12th Grade Immunization Requirements

Your child’s health is important to us at P-H-M! We believe that healthy children are in a better position to take advantage of all the educational opportunities we offer; and immunizations help ensure your child’s health and well being. Immunizations provide vital protection against diseases not only for your child, but also all the children in our community’s schools.

Immunizations may be obtained from your family doctor, St. Joseph County Health Department, or other area clinics. 

The Indiana State Department of Health requires parents/guardians to provide their child’s full immunization record to the student’s school health office on or before the first day of school. Parents can use their health provider or the local health department for immunizations. Click here to visit the IDOH website for a list of local health departments.  In accordance with state law, parents/guardians are required to furnish (no later than the first day of school after your child’s enrollment) proof of your child’s immunization status, either as a written document from the health care provider who administered the immunizations or documentation provided from the state immunization data registry. 

For each school year, parents are asked to please furnish the immunization paperwork before the first day of school in August. Click here to view P-H-M’s school year calendar.  By doing so, this will prevent any interruption to your student’s attendance at the start of the school year. Failure to do so may result in a child’s exclusion from school until the immunization requirements have been met.

Among IDOH required immunizations are the MCV4 vaccine, which protects against Meningococcal Meningitis, and is to be administered to students entering grades 6 and 12. Click here for more information about Meningococcal Meningitis.  

The IDOH has also provided Information to parents about the known link between the Human Papillomavirus (HPV) and cancer; this vaccine is also recommended not required. Learn more about HPV vaccination here. 


Indiana requires that all students have specific vaccinations prior to attending school to minimize student exposure to preventable, life-threatening disease. In the case of medical or religious exemptions, parents must print and fill out the appropriate form and return the form to your child’s school.
