Welcome to Penn-Harris-Madison School Corporation! P-H-M offers full-day kindergarten at all 11 elementary schools, giving our youngest learners a strong start with caring teachers, engaging classrooms and opportunities to grow.

How to Register for Kindergarten

Step 1: Find Your Elementary School

Your home address determines which P-H-M elementary school your child will attend. Use our P-H-M district boundaries map to find the school that serves your address.

Not sure which school serves your address? Call P-H-M at (574) 259-7941 for assistance.

Live outside P-H-M district boundaries?

Your child may still be eligible to attend P-H-M. Learn about eligibility requirements and how to apply.

Step 2: Gather Your Registration Documents

Before beginning Kindergarten registration, have the following documents ready:

• Your child’s birth certificate
• Your child’s immunization records
• Two proofs of Indiana residency, such as a current mortgage or lease statement and a current utility bill

Step 3: Complete Kindergarten Registration

Once you’ve identified your child’s home school, select that school below to access its online registration form. A parent or guardian must complete the registration process.

Prefer to complete a paper registration packet?

Families who are unable to complete the initial registration forms online may print the registration packet and submit the completed forms to their school’s office. The Registration Packet includes:

• Registration Form
• Home Language Survey Form
• Transportation Add/Change/Withdrawal Form

When can my child start kindergarten?

For the 2026–2027 school year, children must be 5 years old on or before September 1, 2026, to enroll in kindergarten at P-H-M.

Can my child attend a different P-H-M elementary school?

Families who live within P-H-M’s district boundaries and would like their child to attend a school other than their designated home school may request an internal transfer.

Incoming kindergarten students must first register at their home school before submitting a transfer request. Approval is based on available space and is not guaranteed.

Before Your Child’s First Day of Kindergarten

Immunization Requirements

Indiana requires families to provide documentation of their child’s required immunizations when enrolling in school.

Submit your child’s immunization records to their elementary school office before the first day of school.

Immunizations may be obtained through your family doctor, the St. Joseph County Health Department or other area clinics.
