Playground Safety Inspection

This is the monthly Playgound Safety Inspection Form required to be completed by the custodial group at each school location having playground equipment.

I have tested stability and rigidity of all climbers, swings, whirls, slides, wood equipment, bouncing rides and sports equipment.
I have checked for proper assembly, installation and ground anchoring
I have checked for protruding bolts and nuts and covered them.
I have checked for bent, broken or severely worn pipe and sharp edges on pipes or plates.
I have filled holes around all equipment and provided a soft, resilient, energy-absorbing ground surface under all equipment.
I have reviewed for exposed concrete footings on equipment.
I have checked to be sure all equipment is spaced away from other equipment and traffic patterns.
I have checked for rusting and worn galvanized frames and painted if necessary.
I have painted playground equipment as needed.
I have checked to be sure all fittings are tight and that the bards and pipes do not move.
I have checked to see if cracking of wood on climbers needs to be filled with silicone caulking
I have checked to be sure the exit of the bedway is parallel to the ground on slides.
I have checked the surface around each slide exit for corrosion.
I have examined the slide bedways, bedrails, handrails for foreign objects, holes and rouch edges.
I have closed and inspected all S-Hooks on swings.
I have tested for free movement of each swing hanger and other moving attached parts.
I have checked for proper grease in tire swing assemblies.
I have checked for worn swing chains and replaced them.
I have inspected and replaced slashed rubber belt seats.
I have checked for wearing away and erosion of the surface around whirl bases.
I have checked for worn bearings and replaced with new one if needed.
A good practice is to replace both bearings at the same time, because if one goes bad, it won't be long until the other follows.
I have checked for loose spring castings and tightened.
I have replaced any broken springs or seats if identified
